In the United States, colorectal cancer is the third most common cancer and the third leading cause of cancer deaths found in men and women not including skin cancer. According to American Cancer Society 103,170 new cases of colon cancer and 40,290 new cases of rectal cancer will be diagnosed in 2012 in the United States. Combined they will cause about 51,590 deaths from colorectal cancer in 2012. According to the New Jersey Department of Health and Senior Services, New Jersey Cancer Registry approximately 1,026 men/women between 2002 and 2007 were diagnosed with colorectal cancer and 353 men/women died of colorectal cancer in AtlanticCounty.

Colorectal cancers begin in the digestive system or the gastrointestinal system. This is where food is changed to energy and waste is rid from the body. The colon absorbs water and nutrients from the food and also serves as a storage place for waste matter. The waste matter moves from the colon into the rectum, the final 6 inches of the large bowel. From there the waste passes out of the body through the opening called the anus during a bowel movement. The colon has four sections. Cancer can start in any of the four sections or in the rectum. The wall of each of these sections (and rectum) has several layers of tissues. Cancer starts in the inner layer and can grow through some or all of the other layers. Knowing a little about these layers is helpful because the stage (extent of spread) of a cancer depends to a great degree on which of these layers it affects. Cancer that starts in the different areas may cause different symptoms.Colonand rectum cancers probably develop slowly over a period of several years. We now know that most of these cancers begin as a polyp–a growth of tissue into the center of the colon or rectum. Polyps are also known as adenomas. Removing the polyp early may prevent it from becoming cancer.

Colorectal Cancer Risk Factors

It is not known what exactly causes colorectal cancer, but certain risk factors increase a person’s chance of getting the disease. This does not mean the person will get the disease nor does it mean people without specific risk factors will not get the disease. It only means there is an increased chance.

Age (Over 50)

Personal History of Colorectal Cancer

Personal History of Polyps

Personal History of Inflammatory Bowel Disease (including Ulcerative Colitis and Cohn’s Disease)

Family History

Ethnic Jewish Background (Eastern European Descent Ashkenazi)

Race (African American)

Diet (High Fat, Animal Sources)

Lack of Exercise

Overweight

Smoking

Heavy Alcohol Use

Type 2 Diabetes

Colorectal Cancer Screening Recommendations

Early detection greatly improves survival of colorectal cancers. Colorectal cancer death rates have been dropping over the last 20 years for both men and women. This is explained by the increased screenings and the removal of polyps before they become cancerous. Screening tests are used to detect cancer before symptoms appear and to remove polyps before they become cancer. It is recommended that after age 50, both men and women should be screened for colorectal cancer yearly and earlier if the person is in a high risk category. There are several options for screening so talk to your doctor about colorectal screening. If you have any symptoms such as a change in bowel habits such as diarrhea, constipation, or narrowing of the stool that lasts for more than a few days, a feeling that you need to have a bowel movement that doesn’t go away after doing so, bleeding from the rectum or blood in the stool, cramping or steady stomach pain or weakness and tiredness be sure to tell your doctor.

According to American Cancer Society 12,710 women in the United States will be diagnosed with invasive cervical cancer in 2011 and 4,290 women will die from cervical cancer in 2011. According to the New Jersey Department of Health and Senior Services, New Jersey Cancer Registry approximately 90 women between 2002 and 2007 were diagnosed with cervical cancer and 37 women died of cervical cancer in Atlantic County. Cervical cancer was once the most common cause of cancer death, but after 1955 and the advance of the Pap, that number has gone down substantially.

The cervix is the lower part of the uterus which connects the body of the uterus to the vagina. Cervical cancer begins in the lining of the cervix and generally does not form suddenly. Cells begin to change from normal to pre-cancer and then cancer. This can take a number of years, although sometimes it happens more quickly. These changes are referred to by several terms, including dysplasia. Sometimes these changes may go away without treatment, but more often they need to be treated to prevent them from changing into true cancers.

Cervical Cancer Risk Factors

Certain risk factors increase a person’s chance of getting the disease. This does not mean the person will get the disease nor does it mean people without specific risk factors will not get the disease. It only means there is an increased chance.

HPV (Human Papillomavirus) – a sexually transmitted disease

Smoking

HIV (Human Immunodeficiency Virus) – a sexually transmitted disease

Chlamydia – a sexually transmitted disease

Diet low in fruit and vegetables

Birth control pills

Having many full term pregnancies

Young age at first full term pregnancy

Poverty

DES (diethylstilbestrol) -a hormone miscarriage drug used between 1940 and 1971

Family History

Cervical Cancer Screening Recommendations

Most cervical cancers can be prevented by avoiding the risk factors and having a routine Pap test (a painless sample of mucus and cells scraped from the cervix). Young women can delay having sex until they are older and protect themselves against sexually transmitted disease by engaging in fewer partners. (Condoms do not protect against HPV, but do help protect against other sexually transmitted diseases.) Women can also quit smoking or never smoke to begin with to decrease risk. American Cancer Society recommends all women should begin having the Pap test within three years after they begin having sex (vaginal intercourse), but no later than age 21 years of age. Conventional Pap test should be performed every year, but liquid Pap tests can be done every two years. A pelvic exam still needs to be completed every year even if your Pap tests are not. A pelvic exam should not be confused with a Pap test. Patients should talk with their doctors about important cancer screenings and should be going to their gynecologist annually as long as there are not problems or symptoms. Any abnormal symptoms should be reported to your doctor right away.

Increasing the Accuracy of your Pap Test:

Do not schedule an appointment during your menstrual period.

Do not douche 48 hours before the test.

Do not have sexual intercourse 48 hours before the test.

Do not use tampons, birth control foams, jellies, or other vaginal creams or vaginal medications 48 hours before the test.

Grant Year: 2010-2011

The Atlantic County Healthy Living Coalition was established in the fall of 2004 and is funded by theNew JerseyDepartment of Health (NJDHSS) and Senior Services, Office of Cancer Control and Prevention. Each year we report the work of the coalition through progress reports to NJDHSS. In order to get the new website up and running quickly we are going to use the fourth quarter progress reports to showcase the work we have completed over the grant years for the past five years. Please see below for details of the 2010-2011 grant year.

Goal: Complete one cancer related event for African American women by June 30, 2011.

It has been decided that the coalition will not complete the annual fashion show. This was decided for multiple reasons including loss of funding. It has been decided that we will do some Friend to Friend Parties for African American women. Friend to Friend Parties are intimate gatherings in which we discuss the importance of a mammogram. Please see below for the events completed during the grant period:

10/21/10: Friend to Friend Party, Pleasantville, 17 women

3/19/11: Friend to Friend Party, Pleasantville, 19 women

We also hosted the New Age Hair and Health Luncheon in Atlantic City. This was an event for the barbers, their families and patrons. The event included education, celebration and honored one of our lost barbers. 30 people attended the event. Mostly fromAtlanticCounty.

Goal: Complete 1 School Based Project

A Road to Good Health is a project that has been taken into the schools. It consists of taking over the gym classes. The class is divided into two in which one side focuses on physical exercise and the other focuses on health education. The students rotate within the sides and then switch half way through to do the other activities. A pre & post survey is completed and the program was recently IRB approved through Rutgers Cooperative Extension. Earlier in the year Joanne Kinsey and Angela Bailey presented at the Superintendent’s Round Table. We sent letters to all middle and high school principals to be delivered in January 2010 upon return to school and received several interested schools. The goal was to get a few more schools to participate. Below are the schools in which the program has taken place during the 2009/2010 school year. 1056 children have gone through the program. A full evaluation is still in progress.

Goal: Complete School Based Project

The Community Education Committee has developed a new teen health fair project for schools. It is a large complex project that is being taken to the schools. The committee wants to pilot it in a couple of schools this year and then potentially take it to more middle schools next year. The age targeted is 7th and 8th graders and would focus on a variety of health topics.

A Road to Good Health

Topics: Nutrition, Physical Exercise, Cancer Prevention, Heart Disease and Diabetes, and Mental Health

The first school was in Northfieldon February 6th, 2009. 535 students were educated. The committee took over the physical education class for the 5th, 6th, 7th & 8th graders. Each class was divided in two. One group participated in a variety of physical exercises while the other group was given a quiz and split in four. Each mini group went to each of the topic tables to find the answers on the quiz. There were a variety of hands on items and discussion at the tables. The two large groups then switched so all the students got to participate on each side. The students turned in their quizzes for a chance to win prizes and the teachers were asked to do follow up projects. The school was very pleased and we learned some good things to tweak.

The second school was VikingAcademyin Atlantic Cityon June 3rd, 2009. It was completed much the same way only it was for 7th through 12th graders. There were 100 students.

Goal: Complete 1 Healthy Lifestyle Event

The Healthy Lifestyle Committee put on the Five-A-Day Olympics this year on August 15, 2008. It featured Olympic style events with a fruit and veggie twist. It was a joint project with the Atlantic County Division of Public Health and it had a turn out of 124 participants representing 38 families. The event featured a variety of Olympic events with a fruit and veggie twist such as Grapefruit Shot-put and Veggie Basketball. There were individual events (two age groups), family events and tot games. There also were cooking demonstrations by Chef Mike, snacks (vegetable packs, fruit kabobs, popcorn, 100% juice water ice and water), trivia signs, balloon animals, medals and giveaways. The event was very easy going with people being able to visit and participate in different activities. It was full, but not overcrowded and the playground was also a nice addition. The event got a nice write up in a couple of papers and families enjoyed a free, fun event. A full evaluation is available and the county park has requested we do the event again.

Goal: State of New Jersey Cancer Awareness Youth Educator Badge.

The badge program has changed this year. Angela Bailey made up educational materials on nutrition, exercise, not smoking and sun safety. Children take a short ten question quiz on the materials and models available. After getting the ten questions they are given a certificate and badge. No information on the children is collected. They are just counted. This activity is offered as an activity for health fairs (especially in the schools) where we expect lots of children. The goal is to do 5 events this year. Five events were completed over the year with 98 patches given out.

8/9/08 – Family Fun Day, EHT – 33 patches

The goal is to screen 800 women for breast and cervical cancer this year. In the first quarter 161 women have been screened for cervical cancer and 79 women for breast cancer. The cervical screening numbers are higher due to younger women having their pap test done but are not age appropriate to have a mammogram screening performed.

The American Cancer Society’s Department of Epidemiology and Surveillance Research is inviting men and women between the ages of 30 and 65 years who have no personal history of cancer to join a historic research study. The ultimate goal is to enroll 500,000 adults from various racial/ethnic backgrounds from across the U.S. The purpose of CPS-3 is to better understand the lifestyle, behavioral, environmental and genetic factors that cause or prevent cancer and to ultimately eliminate cancer as a major health problem for this and future generations.

Study enrollment will take place at Relay For Life events in Medford on June 21 and Mullica Hill on June 6.

CPS-3 is a grassroots effort where local communities from across the country can support cancer research not just through fundraising efforts like Relay For Life, but also by participating actively in this historic research study.
